Looking at this piece, it is hard to believe the people aren't real. From the folds in the skin to the hairs on their legs, they are incredibly detailed. In fact, all of his work (more shown below) are amazingly realistic.
The expression on their faces is mutual; they both look deep in thought, thoughts that aren't necessarily nice.
The fact they are spooning (and both half naked) suggests they are a couple, however his arms aren't reaching around her in an embrace. Although they are together physically, i don't think they are emotionally. They are each in their own headspace, making the overall impression a cold and distant one.
